Birthday Party Liability Waiver & Agreement
By signing below, I acknowledge and agree to the following:

I understand that participation in activities at Omnia Dance Studios, including but not limited to use of the bouncy castle, dance equipment, and studio space, involves inherent risks of injury.

I accept full responsibility for my child(ren)’s supervision, safety, and behavior while on the premises. Parents/guardians are solely responsible for supervising their own children at all times.

I understand that Omnia Dance Studios, its owners, staff, and affiliates are not responsible or liable for any injury, accident, loss, or damage to persons or property occurring during the birthday party event.

I agree to assume all risks associated with participation and use of the studio space and equipment.

I agree that any damage to the studio property or equipment caused by my child(ren) or guests under my supervision is my financial responsibility.
By signing below, I confirm that I have read, understand, and agree to the terms of this waiver.
